Clinical Assessment

A process for evaluating the clinical aspects of an individual’s health condition (including medical, behavioral and psychosocial aspects) that have been identified as eligible for a healthcare management program to identify specific needs related to their clinical condition and associated co-morbidities. For licensed or certified clinical staff, assessment is performed within the scope of practice of the discipline.
